Cílem předmětu je a/ procvičit metody revize farmakoterapie se zaměřením na lékové problémy při maximalizaci účinku a minimalizaci rizik; b/ prohloubit znalosti a dovednosti v klinické farmacii c/ zvážit, která relevantní informace o nemoci a pacientovi jsou nutné (viz 5otázek diagnostiky u chrob a teorie 3pilířů u NÚ konkrétního léčiva nebo skupiny) a zda se nacházejí ve zdravotnické dokumentaci d/učit se komunikovat s pacientem k upřesnění lékové a osobní anamnézy nutné pro minimalizaci rizik lékových problémů e/ učit se komunikovat s lékařem o lékových problémech a způsobech jejich řešení, pokud jsou klinicky významné Výuka je složena z praktických cvičení u lůžka pacienta (27hod); obecné semináře 9hod a prezentační semináře s diskuzí nad kazuistikami z oddělení (pokus o aplikace algoritmu SAZE (18hod). Předmět je vyučován ve 4. úseku LS a vyžaduje aktivní znalosti z Farmakologie, Klinické farmacie 1, Personalizované medicíny a interpretace základních biochemických, hematologických a fyzikálních hodnot a poznatků z farmaceutické technologie týkající se biofarmacie. Studenti využijí i poznatky z Klinické farmacie 2, která bude přednášena paralelně s tímto kurzem a budou zde v obecné části přednášeny i teoretické podklady revize farmakoterapie Všechny tyto poznatky musí student aktivně při výuce využívat a bude na ně odkazováno.

Practical exercises at the bedside will be led by experienced teachers in the field – doctors or clinical pharmacists. Teaching will take place in groups of up to ten people and students will have access to medical documentation, the opportunity to talk to the patient and discuss the issue with the attending physician or clinical pharmacist. Their aim will be to obtain such relevant information, which they will use within the SAFE algorithm to detect signals of risk of drug problems (DRP in maximizing the effect and minimizing the risks, to analyse the risk of DRP (patient sensitivity to risk), to measure the risk and monitor the risk/benefit and how to eliminate the risk by preventive measures or active intervention in the manifestation of risk using clinical-pharmaceutical care tools (how to use the drug, how to use  how to prevent drug nonadherence, the use of monitoring and self-monitoring; regime measures; Proposal for modification of pharmacotherapy, including self-medication – (inappropriate/appropriate). Students in groups of two and three will always be assigned one patient at each practice, whom they will analyse and describe everything in a Word presentation with the patient's characteristics, anamnestic data and fill in the data that require answers to 5 diagnostic questions (5QDg), they will define the goal of treatment and then analyse whether it is possible or have achieved the goals based on 5QDg and using direct and indirect indicators to evaluate the 5th question of the 5QDg method. In order to minimize risks, DRP signals reveal at the level of adverse effects, drug interactions, contraindications and possibly drug errors. They will use the 3-pillar theory of the relationship between the side effect (potential side effect) and the administered drug. These data for drug problems while maximizing the effect and minimizing risks are then solved using the SAFE algorithm, including recommendations on whether to intervene in the method of drug use, drug adherence, monitoring (including self-monitoring), regime measures and a proposal for modification of Rx or OTC pharmacotherapy.  Each student will prepare a drnd detailed structured presentation in PowerPoint and Word (describe the basic data) form that they obtained for the case report in the hospital) to the guarantor of the course. That's 5 case reports/3-member group or 4 case reports/2-member group. Each student will choose one case report for presentation at a colloquium, during which credit will be awarded for attendance; and evaluated the performance of the subgroup for the defence of the case report with proposed solutions.

Zápočet při dosažené docházky (100% nebo 1 kazuistika navíc za každou absenci) a odeslání 5 kazuistik /2členná či 7 kazuistik/3členna podskupina. Všechny kazuistiky budou vystaveny v Moodle

Zkouška proběhne formou kolokvia ve stanovených termínech. Podskupina si vylosuje kazuistiku  a po hodině přípravy ji bude prezentovat dalším dvěma podskupinám a v tomto prostředí proběhne diskuze a aktivita při diskuzi a odpovědích bude ohodnocena známkou.

Pokud se student nemůže zúčastnit kolokvia přihlásí standfardně v době zkouškového na zkoušku a vylosduje si jednu kazuistiku, kterou budou se zkoušejícím řešit.

 U zkoušky obou forem bude  student/podskupina studentů prezentovat vybranou kazuistiku ze zaslaných i ostatními skupinami kurzu a které jsou uvedeny na Moodle; jejímž cílem bude prokázání znalostí a dovedností při managementu rizik farmakoterapie a aplikace algoritmu SAZE a teorie Tří pilířů vztahu ADR a léčiva a nebo skupina léčiv a budou aplikovat individuálně  relevantní nástroje (způsob užití, lékovou adherenci, monitorování, revize farmakoterapie, životní styl a samoléčbu) při maximalizaci účinku (to je dosažení cílů farmakoterapie). Studenti při zkoušce budou testováni, jak umí aplikovat znalosti z farmakologie, farmaceutické péče a klinické farmacie 1 a 2 a biofarmacie do řešení kazuistiky.
