Propedeutics in surgery introduces elementary theoretical and practical aspects of surgery.
Aim of the course -

The aim of this subject is to introduce practical and theoretical aspects of surgery.

A. Seminars theoretically explain following topics:

Introduction, history of surgery, surgical fields and specializations.  Basic terminology, nomenclature in surgery (incision, excision, exstirpation, punctuation, excochleation, reduction, etc.).

Patient examination in surgery. Patient history, physical examination, laboratory diagnostics, basic imaging methods. Case presentation. 

Medical documentation in surgery. Fundamental principles, legal aspects.

Common surgical diagnoses: tumors, cysts, ulcers, sinuses, fistula, necrosis, gangrene. Patophysiology, common location, therapy, technique.

Concomitant diseases and health conditions (metabolic, cardiovascular, ventilatory, psychological and psychiatric), specific aspects (childhood, old age).

Surgical medical devices. Basic instruments, coagulation, suction, turnstile, loupes, microscope, etc. Types of sutures. Types of dressings and dressing material.

Drug application. Injection and distribution, methods of administration, infusion, instillation, infiltration, etc. (oral intake, patches, gel, cream, ointment, eye drops and ear drops).

Vascular system cannulation. Peripheral venous system, central venous system, arterial system, catheterization, technique.

Types of wounds and their characteristics, wound healing (per primam intentionem, per secundam intentionem). Basics of defect closure.

Internal and external haemorrhage, physiological and surgical haemostasis. Transfusion. Indications, application principles, complications. Infusion therapy in surgery.

Operating room management, operating group, operation protocol. Asepsis, antisepsis, types of sterilization and disinfection.

Pre-operative preparation of the patient (local, general), operative field.

Local and general postoperative treatment, postoperative complications – classification, prevention, diagnostics, therapy.

Inflammation and infection in surgery (early, local, systemic, phlegmon, abscess, furuncle, carbuncle, sepsis, anaerobic infections), tetanus prevention, nosocomial infections.

Drainage methods in surgery, principles, indications. Puncture, types, technique, indication.

Types of anesthesia (general, local and regional anesthesia).

Nutrition and Dietetics in surgery. Rehabilitation in surgery.

B. Subject furthermore aims to teach students following practical skills:

Dressing techniques, surgical instruments, types of suture material, sutures and other types of skin closure.

Preparation of surgeon for surgery,  surgical gown and surgical gloves donning.

Basic techniques of wound sutures. Basics of osteosynthesis and instruments (drill, plates, nails, screws).

Skin transplantation, instruments, basics of microsurgery (loupes, microscope).

Dressings: ward and outpatient department.

Assisting a surgeon in the operation room.

Requirements for obtaining the credit from Propedeutics in surgery:

The condition for the credit is 80% attendance of the practices and finalization with an oral examination at the Department of Plastic Surgery 

(answering from the list of given questions and additional knowledge attained in surgical propaedeutics that were 

discussed in the seminars). 

For students in the English curriculum with distant (online) studies, the condition for the credit is attendance of online courses- 

interns in MS Teams (4 of 5), completion of internship at the surgical department of the hospital in the place of residence in the length of 3 

teaching units (4.5 hours, compensation for internships 6-8) and a distant (online) form of examination (see below). 

Students may register for the exam via SIS. 

For students of the distant study group of the English curriculum who are abroad, we will count on distant (online) testing via MS Teams. 

In justified cases, we will also enable distant (online) testing for other students who are abroad 

(possibly quarantined) and do not have the opportunity to attend the exam. In case of applying for distant online testing (justification needed), please send your request to plastsec@lf3.cuni.cz, including your telephone contact. Every request will be assessed individually with a proposal for the exam date. 

Preventive epidemiological measures within social interactions often change. Please keep up to date with the situation via the faculty website before the exam.

All students must come prepared in all white hospital scrubs and hospital shoes to be prepared for contact with patients. ALL STUDENTS WHO DO  NOT RESPECT THIS WILL NOT BE ALLOWED TO TAKE PART IN THE PRACTICE.

Introduction, history of surgery, surgical branches and specializations. Examination of the patient in surgery (anamnesis, physical examination, laboratory examinations, basic imaging methods). Examples of case studies.

The basics of medical records in surgery, including legal aspects.

S2

Basic terms, terminology in surgery (incision, excision, extirpation, puncture, excochleation, reduction, etc). Common surgical nosological aspects (tumors, cysts, ulcers, sinuses, fistulas, necrosis, gangrene - (pathophysiology, anatomical sites of occurrence in the body and methods of treatment), concurrent diseases and problems (metabolic, cardiovascular, ventilatory, psychological and psychiatric), specific aspects (childhood, old age).

Surgical medical devices - basic instruments, devices (coagulation, suction, turnstile, magnifying glasses, loops, microscope). Types of suture materials. Types of dressings and dressing materials. Drug application: injections- distribution and methods of application, infusion, instillation, infiltration, and more (oral intake, patches, gel, cream, ointment, eye and ear drops).

Types of wounds and their properties, wound healing (per primam intentionem, per secundam intentionem). Basics of wound coverage. Bleeding internal, external, physiological and surgical hemostasis. Transfusion. Indications, application principles, complications.

 S5

Operating room management, operating group, operating protocol. Asepsis, antisepsis, means and types of sterilization and disinfection. Preoperative preparation of the patient (local, general), including the actual operating field. Local and general postoperative treatment, postoperative complications-division, prevention, diagnosis, basics of therapy.

 S6

Inflammation and infection in surgery (early, local, systemic, general, phlegmon, abscess, furuncle, carbuncle, sepsis, anaerobic infection), tetanus prevention, nosocomial infections. Drainage methods in surgery, principles, indications. Punctures, types, method of execution, indications.

Types of anesthesia (general, local and regional anesthesia).

Nutrition and dietetics in surgery. Infusion therapy in surgery. Cannulation of the vascular system (peripheral venous system, central venous system, arterial system. Catheterization. Rehabilitation in surgery.

  

Practicals:

 All students must come prepared in all white hospital scrubs and hospital shoes to be prepared for contact with patients. ALL STUDENTS WHO DO  NOT RESPECT THIS WILL NOT BE ALLOWED TO TAKE PART IN THE PRACTICE. 

P1 Scrubbing in. Surgical gowns and gloves, how to prepare. Preparation of the operating field. Training of surgical knotting with the hand - a simple knot.

P2 Surgical instruments, types of suture materials, practicing skin sutures (single and intradermal sutures), surgical knotting with the hand - double knot.

P3 Removal of atraumatic skin suture (single, intradermal sutures), tendon suture, surgical knotting of the hand - single and double knots.

P4 Basics of osteosynthesis, instruments (saw, drill, plates, screws).

P5 Basics of skin transplants, instruments (skin sampling, MESH), basics of microsurgery (magnifying glasses, microscope)

P6 Outpatient, Trauma ward, operating rooms, departments: dressings, assistance in operations and outpatient
