Missed practice sessions can be replaced only after individual agreement of a student with the senior teacher of the cardiology department where the practice was missed and will depend on availability of our doctors due to their clinical duties.

Spirometrie:
Podmínkou klinického zápočtu je potvrzená účast na alespoň 12 z celkového počtu 15 klinických praktik (kardiologie a pneumologie).

Praktické dovednosti jsou doporučené, ale nejsou povinné.

Organization of course Cardiology, pneumology, cardiac surgery and angiology; Year 4
1. This course forms a significant part of tuition in internal medicine - cardiovascular and pulmonary fields. Emphasis will be placed on the repetition of propedeutics and on gaining experience in diagnostic methods, differential diagnosis, and the basics of therapy. We believe we have prepared an interesting block, there is a recent electronic textbook available (http://www.lf3.cuni.cz/cs/pracoviste/3interni/vyuka/elektronicke_srdce_a_plice/) and a nice ECG textbook was attacked by hackers in the past, but has been repaired and is available for you (www.ucebnice-ekg.cz), but unfortunately only in Czech. 2. Participation in this course must be preceded by successful passing of the exam „Basics of general pathology and pathophysiology" in Year 3. This will be verified before the start of our course in SIS; any student not fulfilling this condition can not participate in this course.
3. Practice sessions (bedside teaching) are compulsory, seminars are strongly recommended but not compulsory and lectures are not compulsory.
Practical information: for a clinical part please bring hospital shoes, coat, name badge, stethoscope, ECG ruler may be helpful.
Practice sessions at the Department of Cardiology will be organized in small groups of 2-3 students and distribution of students into specific sites will be available.
Pneumology is taught at University Hospital Bulovka. All parts of this course take place at Pavilion 4 on the hill, it is opposite the Proton Centre. Meet always by the Lecture Hall – 2nd floor on the left.
4. Credit conditions – there are 2 credits, both are independently entered into SIS and your logbook (if you use it). Should a student fail in any of those credits he will need to repeat the whole course. a. Written multiple-choice test from the pre-clinical part, this test will be formed by 30 questions from pharmacology, 30 questions from pathology and 30 questions from pathophysiology. Every test will last 90 minutes in total. Evaluation "passed" requires the correct answer of at least 70% of questions in every field and at the same time at least 75% of all questions. This test is evaluated overall; therefore, even a student who failed in one field only must repeat the whole test. The pre-clinical test will be scheduled at the end of every course block; this test is open only to students of this course; no other students will be allowed to sit it. Further tests will be scheduled: 2 in the winter examination period, 2 in the summer examination period and 1 in September – there will be no limit on the number of students. Students who fail the test 3 times will be examined orally by a panel formed by examiners from fields of pharmacology, pathology, and pathophysiology. In case this is needed one date will be scheduled in September.

b. Clinical credit – there has to be 80% presence on practical teaching sessions with the signed logbook (at least 12 signed practise sessions) plus the presentation of a correctly written admission patient admission notes including working diagnosis plus basic differential diagnosis and also to demonstrate physical examination at the patient bedside (or by a simulator). This credit is organised by the Department of Cardiology at the end of every course and participation is compulsory. In case of absence, the student must agree on the replacement credit slot individually with the assigned examiner. This clinical credit can be taken 3 times only.
5. Examination is organized during the examination period only. Successful passing of both above-mentioned credits is necessary to register for the examination in SIS. The examination is oral and the list of questions is available in SIS. There will be enough slots available during the examination periods and also in September. We recommend passing this exam early.
This exam has 2 parts: part 1 consists of ECG description – faultless description is needed to proceed to part 2 and incorrect ECG description results in “failed” classification; part 2 is one question from cardiology and/or pneumology, always including clinically relevant pathology, pathophysiology, pharmacology, clinical propaedeutics, diagnostic methods and if applicable basic therapy. There will be an emphasis on clinical propaedeutics (history and physical examination); poor knowledge will result in classification "failed" even should the other knowledge be acceptable.
6. Students, who studied this subject abroad (i.e. Erasmus programme), will present a document proving their participation including the extent of practical bedside sessions and may receive clinical credit based on this document. All students must pass the preclinical test and final exam organized by 3.LF UK.
7. Teaching specifications during the COVID-19 pandemic:
a) Practice sessions will still be organized in a physical form and are compulsory. Precautionary measures will be followed per currently valid recommendations by Charles University. Every student will sign the declaration: http://covid19.lf3.cuni.cz/cestne-prohlaseni/. Students must have covered their mouth and nose with an FFP2 respirator or surgical mask (i.e. not cloth mask).
b) Should any student not be able to participate in all practical sessions due to objective reasons - i.e. not able to travel to the Czech republic at all, please contact the subject guarantor below and you will be allowed to arrange a clinical practice at an alternative hospital and time. The acknowledgement of this arrangement must be agreed between the guarantor and the teacher responsible for your education, who will be required to fill in your logbook per SIS.
c) Seminars form the knowledge base of this subject and define the minimal extent required to succeed at the exam. Participation in seminars is strongly recommended but not compulsory. Seminars will be presented in person.
d) Lectures form subject extension into every lecturer interesting or favoured topics. All lectures will be presented in person.
e) Preclinical test, clinical credit and final exam will be preferentially organized in person but might also be available online per current epidemiological situation
f) all online teaching in this subject will use the platform Meet 3.LF UK, the online preclinical test will also use the 3. LF UK system.
g) The epidemiological situation is subject to a change and the organization of this subject might need to adapt accordingly. Please, do follow your email inbox which you entered into SIS – we shall use this for all future communication.

Cardiovascular and pulmonary diseases are together the most frequent cause of mortality and morbidity and cause decreased quality of life for our patients and significant financial cost for the health-care system. Their symptoms are often similar (for example chest pain and dyspnoea) and this course is therefore very suitable for a problem-oriented approach to teaching. There is close cooperation of pre-clinical and clinical specialists and this allows us to offer a complex and multi-faceted way of education.
1) Practice sessions at the bedside.
Every student has to participate in at least 12 clinical practice sessions. The practice schedule ensures a balanced student distribution among all clinical stations.

Choroby kardiovaskulární a plicní jsou dohromady určitě nejčastější příčinou úmrtí a jsou zátěží pro pacienty, kterým zhoršují kvalitu života i finanční zátěží pro celý zdravotní systém. Nejčastější symptomy mají nemoci obou těchto orgánů velmi podobné (například bolest na hrudi či dušnost) a tento předmět je tedy velmi vhodný pro problémově orientované pojetí výuky. V rámci tohoto předmětu úzce spolupracují kliničtí a prekliničtí specialisté a nabízíme tedy studentům celostní a komplexní přístup k výuce.

Should a student fail in any of those credits he will need to repeat the whole course. a. Written multiple-choice test from the pre-clinical part, this test will be formed by 30 questions from pharmacology, 30 questions from pathology and 30 questions from pathophysiology. Every test will last 90 minutes in total. Evaluation "passed" requires the correct answer of at least 70% of questions in every field and at the same time at least 75% of all questions. This test is evaluated overall; therefore, even a student who failed in one field only must repeat the whole test. The pre-clinical test will be scheduled at the end of every course block; this test is open only to students of this course; no other students will be allowed to sit it. Further tests will be scheduled: 2 in the winter examination period, 2 in the summer examination period and 1 in September – there will be no limit on the number of students. Students who fail the test 3 times will be examined orally by a panel formed by examiners from fields of pharmacology, pathology, and pathophysiology. In case this is needed one date will be scheduled in September.

b. Clinical credit – there has to be 80% presence on practical teaching sessions with the signed logbook (at least 12 signed practise sessions) plus the presentation of a correctly written admission patient admission notes including working diagnosis plus basic differential diagnosis and also to demonstrate physical examination at the patient bedside (or by a simulator). This credit is organised by the Department of Cardiology at the end of every course and participation is compulsory. In case of absence, the student must agree on the replacement credit slot individually with the assigned examiner. This clinical credit can be taken 3 times only.
5. Examination is organized during the examination period only. Successful passing of both above-mentioned credits is necessary to register for the examination in SIS. The examination is oral and the list of questions is available in SIS. There will be enough slots available during the examination periods and also in September. We recommend passing this exam early.
This exam has 2 parts: part 1 consists of ECG description – faultless description is needed to proceed to part 2 and incorrect ECG description results in “failed” classification; part 2 is one question from cardiology and/or pneumology, always including clinically relevant pathology, pathophysiology, pharmacology, clinical propaedeutics, diagnostic methods and if applicable basic therapy. There will be an emphasis on clinical propaedeutics (history and physical examination); poor knowledge will result in classification "failed" even should the other knowledge be acceptable.
6. Students, who studied this subject abroad (i.e. Erasmus programme), will present a document proving their participation including the extent of practical bedside sessions and may receive clinical credit based on this document. All students must pass the preclinical test and final exam organized by 3.LF UK.
7. Teaching specifications during the COVID-19 pandemic:
a) Practice sessions will still be organized in a physical form and are compulsory. Precautionary measures will be followed per currently valid recommendations by Charles University. Every student will sign the declaration: http://covid19.lf3.cuni.cz/cestne-prohlaseni/. Students must have covered their mouth and nose with an FFP2 respirator or surgical mask (i.e. not cloth mask).
b) Should any student not be able to participate in all practical sessions due to objective reasons - i.e. not able to travel to the Czech republic at all, please contact the subject guarantor below and you will be allowed to arrange a clinical practice at an alternative hospital and time. The acknowledgement of this arrangement must be agreed between the guarantor and the teacher responsible for your education, who will be required to fill in your logbook per SIS.
c) Seminars form the knowledge base of this subject and define the minimal extent required to succeed at the exam. Participation in seminars is strongly recommended but not compulsory. Seminars will be presented in person.
d) Lectures form subject extension into every lecturer interesting or favoured topics. All lectures will be presented in person.
e) Preclinical test, clinical credit and final exam will be preferentially organized in person but might also be available online per current epidemiological situation
f) all online teaching in this subject will use the platform Meet 3.LF UK, the online preclinical test will also use the 3. LF UK system.
