Annotation -
The subject Clinical Pharmacy I (CP I) focuses on basic knowledge of clinical pharmacy, essential for the follow-up subject of the specialized undergraduate branch of clinical pharmacy called Clinical Pharmacy II (CP II) and for the utilization of knowledge in pharmacy/pharmaceutical care in the subject Pharmaceutical Care II (PC II). The main content of Clinical Pharmacy I subject is to focus on ethiopathogenesis and epidemiology of basic diseases in clinical practice, risk factors including drug causes, diseases manifestations (symptoms, basic diagnostic tests/assessments), complications of untreated diseases, goals of therapy, treatment strategies in various clinical situations (non-pharmacological, pharmacotherapeutic strategies - major indications, contraindications of drug groups/drugs), and repetitive knowledge from pharmacology of class and individual properties of medicines, and new knowledge on drugs risk/benefit in the main clinical situations and absolute and relative contraindications of medications in clinical practice. The subject trains the ability of students to assess at the basic level appropriateness of the indicated drugs/drug regimens and to evaluate the basic rationality of pharmacotherapy (from the point of view of appropriate indications and contraindications of frequently used medications) by taking into account patients´ comorbidities and standard clinical recommendations of evidence based medicine. The subject Clinical Pharmacy I (CP I.) is followed in undergraduate specialized branch of clinical pharmacy by the subject Clinical Pharmacy II (CP II), which deepens knowledge on the principles of the individualizations of drug schemes in specific groups of patients (eg. in patients suffering from renal and hepatic failure, in geriatric and pediatric patients etc.), with an emphasis on the use of knowledge from both subjects CP I and CP II in the individualization of drug schemes in clinical practice. Study materials, information about the time schedule of lectures and seminars and any changes in the lectures schedule are sent by the teacher of the English branch of subject Prof. Jiří Vlček (vlcek@faf.cuni.cz) to subscribed students via SIS and by e-mail communication. Teaching is in the form of lectures, interactive seminars and individual preparations of students, including works on a clinical case report, submitted before examination according to the instructions of the teacher. Credit (condition to pass the credit and to have it registered in the SIS)

a/ Presence on seminars

-          70% of presence on seminars is requested. If not fulfilled, it will be necessary to prepare one additional clinical case more and to present it to the teacher – missing 3hours = 1 clinical case

b/ Successful passing of a credit test 

 credit test contains: aims, risk factors of disease, risk factors of complication, basic diagnostic methods + strategy of disease management (pharmacological, non-pharmacological) discussed during the seminars

c/ preparation of your own clinical case and discussion on it (usually ill relative, friend, etc.). Evaluated is also the knowledge of the mechanisms of action of medicines, their major indications and contraindications, what are the goals of the treatment and which of them are reachable and how. The same case (improved after the credit exam) will be discussed also  during the oral examination.

 Oral exam – condition for passing the exam and registration in SIS

1/ only students with fulfilled prerequisites can register for oral examination in SIS

2/ passed credit from clinical pharmacy 1 is necessary condition

 

Consultation before exam is scheduled in any time during the time or after the lectures and on January 7th and 8th , 2025, as emphasized in the study plan of lectures and seminars.

If students need additional consultation, prof. Vlček is willing to offer them individually according to the needs of students, but without payment for students participating on at least 70% of lecturesl, rest should pay each hour of consultation and max 1,5 hour of consultation is offered.

Diagnosis (the student is able to answer 5 questions: 1/ does the patient suffer to disease; 2/ what is the severity of disease and are there present complications of a particular disease; 3/ are there present risk factors and triggers of a particular disease? 4/ are there presented risk factors of complications? 5/ does the treatment reaches the aim? Which diagnostic methods help us to answer these questions?

Aim of treatment

Strategy of the treatment of the acute and chronic form of disorders (focus mainly on pharmacotherapy, but the students should know also major non-pharmacological methods of the treatment)

Pharmacotherapeutic strategies of the treatment (therapeutic properties, pharmacology, evidence based risk/benefit of these methods, populational experience  are additional information that can the discussed also to explain the role of pharmacotherapy);

2/ Question No 2: Main indications and contraindications of selected group of drugs, incl. theoretical explanations of mechanisms of therapeutic effect and side effects, class effects and individual properties of medications. Questions focuses on medicines used for the treatment of disorders stated above (studied during the lectures and seminars)  

Not all topics for exam are going to be lectured, we believe that you can self-study from different sources and from the past lectures at the University also. During consultation, you can ask prof. Vlček id your study materials and way of studying is appropriate. 

For Erasmus students without prerequisites for Clinical pharmacy the time of the exam will be enlarged for additional 30 minutes to test students pharmacologic knowledge necessary for selected topics of question 1 and 2. Self-study of pharmacology properties of medicines used in major disorder lectured is required, sometimes consultations at the Department of Pharmacology are necessary as well. Good knowledge from pharmacology is necessary condition for passing the subject Clinical pharmacy (students should show  broader knowledge regarding basic pharmacodynamic an pharmacokinetic properties of medicines of interest and knowledge of their class effect).

Requirements to the exam -

Oral exam

For the oral exam can register only 

1/ students with prerequisites

2/ students successful in passing credit from the subject Clinical pharmacy I.

 Consultation before exam is scheduled at any time during lectures and on January 7th and 8th as stated  in the schedule of lectures and seminars.

If students need additional consultation, prof. Vlček offer them without payment only for students participating in at least 70% of lectures and seminars, the other should pay each 1hour of consulation (max time for 1 consultation is 1,5 hour). 

Oral examination

Main study source are pdf/word files of lectures- and seminars sent by prof.Vlček before each lecture/seminar. Then are recommended also a textbook of Clinical pharmacy (see section Literature) and current guidelines on the treatment of various disorders.

Learning outcomes -

The subject Clinical Pharmacy I is a main, integrating, clinical subject that connects clinical, epidemiological and pharmacological knowledge about diseases, the therapeutic process, the patient and drugs and uses the principles of determining population and individual efficacy and safety of pharmacotherapy in individualized treatment. The subject builds on the knowledge and skills acquired by students in previous therapeutically oriented subjects: Human Morphology and Physiology, Immunology, Pathological Physiology for Pharmacists, Microbiology, Molecular Biology and Genetics, Toxicology for Pharmacists and Pharmacology.
The prerequisite for the subject Clinical Pharmacy I. is the completion of the subject Pharmacology. The subject Clinical Pharmacy I. is a prerequisite for the specialized branch Clinical Pharmacy, especially the subjects Clinical Pharmacy II and Clinical Pharmaceutical Care, which allow for a deeper look into clinical pharmaceutical knowledge and skills needed on the path of growth towards the future profession of a clinical pharmacist. The subject Clinical Pharmacy I is also a prerequisite for the subject Pharmaceutical Care II, which trains basic knowledge and skills in the field of medication counseling, focused primarily on the effective and safe use of medications and medical devices by patients, support of patients for rational self-medication treatment, and rational implementation of home monitoring.

Based on the acquired knowledge, the student will be able to:
 define the importance of clinical pharmacy as a field and the need for clinical-pharmaceutical care in various healthcare settings
 describe the levels of education in clinical pharmacy and professional organizations supporting the development of clinical pharmacy
 explain the epidemiology of the occurrence of selected basic diseases, their etiopathogenesis, main risk factors (non-drug and drug-related, controllable and uncontrollable), basic diagnostic methods (symptoms, most frequently used diagnostic tests, methods), subtypes and stages of the disease, basic complications of the disease and will be able to use this knowledge in assessing the patient's risk and determining the basic goals of therapy in clinical case reports
 determine the basic goals of therapy and especially pharmacotherapy, namely populational (for nosological units, or their subtypes) and individual (based on the patient's clinical case, in basic aspects)
 explain the main strategies for treating the most common diseases (strategies for treating acute and chronic forms, with an emphasis on pharmacotherapeutic strategies and basic knowledge of the main non-pharmacological procedures)
 describe the basic indicated drugs/drug combinations recommended according to the EBM ("evidence based medicine") in various stages of chronic disease and in acute situations, also for the treatment of complications of the disease
 estimate the basic expected efficacy and basic risks of pharmacotherapy in a specific clinical situation, using pharmacological knowledge (about the mechanisms of desirable and undesirable effects of drug groups and drugs, pharmacokinetics and pharmacodynamics of drugs), and pharmacoepidemiological and clinical knowledge
 evaluate and interpret basic available data about the patient, including very basic laboratory results and clinical data, estimate the need for additional information (clinical and other) for a relevant assessment of the basic efficacy and safety of pharmacotherapy
 assess the appropriateness/inappropriateness of pharmacotherapy at the level of basic indications, contraindications, possible main adverse effects and basic drug interactions (knowledge of dosing is not required at this stage!)
 suggest the main procedures for monitoring the achievement of therapeutic goals, for monitoring complications, control of risk factors, as well as for monitoring the expected main benefits/main risks of pharmacotherapy. Is able to estimate the probability of achieving/not achieving the therapeutic/pharmacotherapeutic goals
