Annotation -
The subject provides targeted information about the physiological and pathophysiological processes, which assert during the human nutrition and which can influence the development of health of individual person. With this commentary is connected the systematic part, which mentions the application of food supplements or food of new type, their legislative position in EU countries and legislative specifics in Czech Republic. They are browsed the individual substances and group of substances (natural, biotechnological and synthetic origin), which mostly do not have the character of drug, and which are presently commonly used in the EU territory and especially as a precautionary preparations, which could protect against the development of civilization diseases and their pathogenesis (oxidative stress, general adaptation syndrome, cancerogenesis), or are used in the certain live periods, but also like the tool, which is possible to use as a adjuvant with the effective pharmacotherapy, without bringing the negative interactions. There is also the information about the preparations which are attractive, which are not authorized in CR (EU), however, in other countries are commonly used (especially USA, China, Japan). As a part of the subject there is the caution on the substances, which commonly occur in food and can affect health or pharmacotherapy, their safe dosages, important interaction or side effects.

Requirements to the exam -

Written examination consists of open questions and multiple-choice questions. To pass this examination successfully, it is necessary to achieve at least 50% of the total points. The questions concern indication, important adverse effects or contraindications, and use specificities of nutraceuticals. The test also checks a knowledge of doses and interactions of chosen nutraceuticals, as well as answers from set of questions (all available at web - study materials).

Syllabus -

Introduction and definition of the topic

  • definition of food supplements and food supplements,
  • differences between the drug and the food supplement,
  • use of food supplements as preventive means,
  • validity of chemical substances (total mixtures of substances) that are not based on chemical drugs,
  • requirements for the so-called biological effect,
  • the difference between nomenclature categories,
  • legislation in the EU, related regulations in the Czech Republic, current state,
  • hygienic standards of some substances whose important nutraceuticals.

Antioxidants

  • pathophysiology of oxidative stress (RNOS),
  • the importance of oxidizing substrates in the body, their positive and negative effects on human tissues,
  • the basis for the development of diseases of civilization,
  • the possibility of interfering with the metabolism of RNOS at various levels,
  • systematics of used substances.

Adaptogens

  • definition of general adaptation syndrome,
  • analysis of pathophysiological consequences of stress,
  • possibilities of intervention at selected levels,
  • systematics of used means.

Substances for calming or stimulating the central nervous system

  • analysis of the pathophysiological condition,
  • enumeration of levels at which the intervention can be performed,
  • substances and plants use, their doses and interactions with pharmacotherapy.
  • substances used in neurodegenerative diseases (Alzheimer's and Parkinson's disease; pathophysiology of processes, possibilities of adjuvant use of substances, substances used, their doses and interactions with pharmacotherapy)

Substances used in chronic fatigue syndrome (CFS)

  • pathophysiology of CFS,
  • systematics of used compounds and natural mixtures.

Immunostimulants

  • definition of non-specific and specific immunity, possibilities of intervention,
  • pathophysiology of immunodeficits,
  • possibilities of use of nutraceuticals, substances and plants use, their doses and interactions with pharmacotherapy.

Substances affecting the risk of neoplasms (chemopreventives) and used within the treatment of cancer

  • pathophysiology of carcinogenesis,
  • influencing by external conditions (exposure to harmful factors of the external environment, diet, etc.),
  • the possibility of intervention with nutraceuticals in the prevention of neoplasms and slowing the progression,
  • the possibility of adjuvant action of nutraceuticals in chemotherapy, radiotherapy, increasing their effectiveness or reducing adverse effects on other tissues,
  • substances and plants use, their doses and interactions with pharmacotherapy.

Substances for influencing the metabolism of diabetes and affecting body weight

  • analysis of factors influencing the development of type 2 diabetes mellitus, overweight and obesity,
  • the possibility of interfering with unwanted development by the action of nutraceuticals,
  • substances and plants use, their doses and interactions with pharmacotherapy.

Substances used in functional damage to the cardiovascular system

  • pathophysiology of cardiovascular diseases,
  • analysis of disease states in which the use of nutraceuticals as preventive or adjuvant agents or as part of therapy is possible.

Substances used for tissue protection (of eye, skin and germinal tissue)

  • hepatoprotectives, substances used in the protection of germinal and ocular tissues
  • the possibility of intervention in undesirable processes by the action of nutraceuticals,

Substances to increase the hygiene of the digestive and urogenital system

  • brief analysis of digestive processes, metabolism and disharmony,
  • substances and plants use, their doses and interactions with pharmacotherapy.

Substances used during menopause and andropause

  • analysis of hormonal conditions during menopause and andropause (pre- and post-climacteric conditions, benign prostatic hyperplasia, loss of libido),
  • substances and plants use, their doses and interactions with pharmacotherapy.

Předmět Potravní doplňky-nutraceutika - GF252 navazuje na znalosti a dovednosti získané v předmětu Farmakognozie II a Farmakologie I a jejich předcházejících předmětů.
Studující po absolvování předmětu umí použít následující termíny nutraceutikum, doplněk stravy, potravina nového typu, tradiční léčivý přípravek, léčivý přípravek.
Výsledky učení:

Studující na základě získaných znalostí a dovedností:

  • vysvětlí rozdíly mezi jednotlivými produkty obsahující rostlinné části či jejich extrakty, vitaminy, minerály a další látky (přírodního i syntetického původu) v uvádění na trh, požadavků na jejich kvalitu, bezpečnost a účinnost
  • orientují se v základní národní legislativě týkající se doplňků stravy a jejímu vztahu k evropskému právu (které typy produktů jsou evropským právem regulovány harmonizovaně a které jsou v gesci národní) 
  • specifikuje podmínky použití zdravotních tvrzení týkajících se doplňků stravy a vysvětlí rozdíly oproti léčebným tvrzením a rozezná k jakému typu příklady tvrzení patří a zda vyhovují platné legislativě
  • vysvětlí, které instituce na národní a evropské úrovni jsou zodpovědné za legislativní činnost, kontrolní činnost nebo mají roli jako poradní instituce a jakým způsobem probíhá uznávání nových surovin pro použití do doplňků stravy
  • dokáže navrhnout specifickým skupinám populace vhodné doplňky stravy pro ovlivnění fyziologických funkcí s ohledem na bezpečnost a účelnost použití s přihlédnutím k osobní anamnéze a/či současné farmakoterapii a upozornit na limity použití nutraceutik
  • zná u vybraných nutraceutik klinicky podložené interakce s léčivy či jinými nutraceutiky
  • zná u vybraných nutraceutik běžně používané (účelné) dávky nebo aplikační formy, případně významné nežádoucí efekty
  • dokáže doporučit nutnost konzultace zdravotního stavu s lékařem či upozornit na rizika oddálení správné konvenční diagnostiky či léčby
