Annotation -
This subject give oportunity to apply student´s knowledge from clinical pharmacy and how to use them in hospital settings in clinical pharmacy care. It will requred middle level of knowledge from clinical pharmacy only. Deep knowlege will be required by state exam in specialisation of clinical pharmacy. Therefore it is recomended for this partucular specialization. But we recomend this subject for specialization of social harmacy, while it is focussed on management of clinical pharmacy care as well and so to have possibility to study another tool to protect rational use of medicines.

Oral examination

Student will receive two topics and he/she will work prepare literature search – one for management of DRP and risk minimization process in hospital. Last step will be case from inpatient.

1/ clinical pharmacy in hospital setting – evaluation, temporary models

2/ clinical pharmacy and payer – evaluation, temporary models

3/ different specialists in hospital and their characteristic

4/ reimbursement of hospital care – models and domestic situation

5/ medicine and allergic anamnesis – sources, formats, how to use them to reach a goal clinical pharmacy care

6/ personal history sources, formats, how to use them to reach a goal clinical pharmacy care

7/ How to use lab data (biochemistry, hematologic) and data from physical examination for maximization of effect and minimization of risk

8/ Using of medicine and personal history by admission – minimization common error on this phase of care

9/ basic medication errors in hospital setting

10/ possible drug related problems by elderly and their minimization

11/ possible drug related problems in pediatric patients and their minimization

12/ possible drug related problems by patients with kidney failure and by hemidialysis and their minimization

13/ possible drug related problems by patients with oncologic disease (solid, hematologic, dermatologic)

14/ Principles of pharmacotherapy review by inpatients

15/ drug related problems of biological treatment in rheumatology, gastroenterology and dermatology

16/ Drug related problems by palliative treatment

Syllabus -

The goal of subject of clinical pharmacy care (CPC) is to improve knowledge and skills how to practice pharmaceutical care in hospital care, principles of management CPC, scientific background of it. Second goal is to exercise how to use data from impatient during his (her) stay in hospital. Clinical pharmacy care can be running for outpatients – we would like to focus on this subject to inpatients only and the students has opportunity discuss similar topics regarding outpatient is different subjects and mainly in Pharmaceutical care.
The subject CPC is focusing on implementation of scientific, historical and management aspects of CPC into the health care of hospital settings. It will be discussed principles of management of CPC hospital care. By the way is opportunity to discuss principles of management of hospitals, sharing of duties and responsibility by different professionals.  It will be discussed different international models of relationships CPC to payers. It will be discussing algorithms of CPC for maximization of effect and minimization of risk during hospitalization and by prevention of risk during process of admission.
The teaching will focus on aspects of documentation of health care and how to manage it and how to protect data privacy use them for forensic reason, continuity , feedback and maximization and minimization of risk of pharmacotherapy in hospital settings and in process of admission and discharging. It will be discussed how to document clinical pharmaceutical care activity to management of particular hospital and to particular payer as well.
It will be discussed relevant data for maximization and minimization process and their misinterpretation regarding medication; allergic, personal, family histories, lab results and data form different diagnostic methods by individual patients and link them with EBM. The students will look for knowledge and skills how to solve DRP by elderly, children, patients with kidney or liver failure, patient with swallowing failure and with metabolic disorders.
