Annotation -
The goal of the subject is to receive skills and knowledge how to provide pharmacotherapy review with aim to optimize pharmacotherapy in sense of rational pharmacotherapy (maximization of pharmacotherapy and minimization risk of pharmacotherapy). The students must learn which data from medical documentation and from communication with patient are important to discover drug related problem and how to communicate it with physicians and clinical pharmacist. They receive from them feedback about their way to use and evaluate available data, which data are necessary and solve drug related problem. The cases will be finished as a home work to be able to study more detailed clinical pharmacy of particular disease and used medicine and make structured report about drug related problems regarding maximization of effect and minimization of risk by SAFE algorithm , 5Q Dg and 3 pillar theory and put it on the word file/or PowerPoint and to present them in from of another colleague In interactive seminars students will focus on individual risk management theory e.g. 3 pillar theory (3PT) of I/ circulation failure; (1/ orthostatic hypotension, 2/ bradycardia, 3/ Torsade de Pointe) 3PTII/minerals failure of K, Mg; Na; Ca; III/ endocrinologic failure (hypoglycaemia, hyperglycaemia, hypothyroidism), case from geriatric department; IV/ dyspepsia and organ failure V/ CNS + neurologic failure. Education contains bedside teaching in small groups (max.10 students) on the ward (30 h); seminars (32h) incl. presentation of cases. Subject is located in summer term of fourth class after finishing subject, which create majority of theoretical knowledge and skills for this disciplines: Pharmacology, Clinical pharmacy 1, Personalized medicine; Interpretation of basic results from clinical examination of patients (from 2018/19) and biopharmaceutical knowledge from pharmaceutical technology.

Course completion requirements -

Exam: student will select one topic concerning  “three pillar theory”: 1/ mineral disturbances, 2/ orthostatic hypotension, 3/ bradycardia, 4/ hypoglycaemia, 5/ hypothyroidism, 6/ diarrhoea, 7/ obstipation, 8/ peptic ulcer, 9/ falls, 10/ esophagitis;  11/ depression, 12/ dementia, 13/ Parkinson sy, 14/ urine infection + 1 case from bed site teaching  by SAFE algorithm approach. The case will be selected from case what the students was working on

Requirements to the exam -

Student receive credit by full attending all seminars and was accepted 6 cases  and active participation on seminars

Subject finish with oral exam. Student will present one of sending case and will show knowledge (theory) and skills how to minimize risk and maximize the effect and application of algorithm  SAFE and algorithm “Three pillars” and will apply relevant tools for prevention and treatment: 1/ how to use medicines; 2/ medication adherence; 3/revision of pharmacotherapy; 4/ monitoring of effect of medicines and behavior or patients; 5/ lifestyle; 6/ self-medication with OTC drugs and food supplements. Exam will linked with Clinical pharmacy 2.

Syllabus -

Bedside teaching is managing on different internal department and on psychiatric department. The students have chance to study medical documentation of selected patients, to select relevant data to be able to evaluate rationality of pharmacotherapy. They have opportunity to talk with some selected patient to learn how to excavate subjective data for analyzing of potential drug related problems. Tutors on ward are serving attending physicians and clinical pharmacists. During this stay each group will prepare 6 cases/3 students and group 5 cases/2 students and will analyze pharmacotherapy and do risk minimization analysis with  algorithm “SAFE” and individual ADR with theory of  “3 pillars” and prepare detailed written version of their analysis (including theory of identified problems) in word and power-point presentation. Both will be send to main tutor of course. In seminars students train 3 pillar theorry of selected adversa drug reaction and relevant medicines

On the seminars they will discussed with main tutor of course some drug related problems and how to solve them (with help of voting system)  and later on each particular group of students will present their cases and their proposal for risk minimization and analysis of rational pharmacotherapy will be discuss in around rest of students from their study group.

Learning outcomes

 Clinical Pharmacy care

Clinical Pharmacy care is an elective course that requires students to revise their knowledge of pathophysiology, pharmacology and clinical pharmacy 1. Students are divided into teams that work together in outside of the frontal education as well and process case reports and Theory of 3 pillars of adverse effects (AD) vs active substance or group of drugs durind seminas in PowerPoint presentation ehat will be discuss into seminars.

The course will enable students to

1/ consolidate the basic knowledge of clinical pharmacy 1 – it is to be able to construct a short characteristic of the disease and its pharmacotherapy, to learn to generate methods of information necessary to assess the maximization of the effect on the basis of 5 questions of diagnostics according to the knowledge that students gain by studying the pathophysiology of the disease.

2/ to expand the knowledge of the process of risk minimization and methods to minimize the risk of the 1st – 3rd order

3/ learn searching for information necessary to minimize risk of 3rd order on the basis of the Theory of the 3 pillars of ADRs of a specific active substance or group of drugs

4/ acquire skills on how to generate a signal and evaluate drug problems (DRP) in the form of a algorithm SAFE. To estimate whether we have enough information to assess the achievement of treatment goals (whether the potential has been achieved or is achieved – see related to 5 diagnostic questions) and whether we have enough information to assess the occurrence or prevention of NUL. These skills will be practiced on individual cases.

5/ learn to work with knowledge and skills published in order to maximize the effect and minimize the risks.

6/ learn to think about the possibilities of changes in pharmaceutical care tools to maximize the effect and minimize risks: 1/ How to use a drug; 2/ drug adherence; 3/ monitoring and self-monitoring of the patient; Proposal for modification of Rx and OTC pharmacotherapy and regime measures

7/ Understand medical documentation and extract information necessary for the evaluation of the 5 questions Dg and the Theory of the 3 pillars of ADR vs pharmaceutical.

8/ try to communicate with the patient to find out the personal and drug history

9/ try to communicate with the doctor and clinical pharmacist to complete the information necessary for the evaluation of the patient's drug problems

110/ summarize the information ad 8/ - ad 10/ and process the case report in a team in Power Pont and then present it on the first case report, credit case report,

11/ try to process a case report with the help of information sources on a randomly selected case report and present it to defend it in front of the other teams.
