Education in the basic specifics of rational geriatric pharmacotherapy
Due to the exponential aging of the population and the expected increase in the number of seniors in the Czech Republic and other countries to 30 % or more by 2050, there is a growing need to significantly increase the main knowledge, skills and competences in the field of safe setting of combined drug regimens in older age and in rational geriatric pharmacotherapy. This course focuses on participants´ modern knowledge and skills in the specifics of rational geriatric treatment and individualized geriatric pharmacotherapy. It increases the competence their competencies in the area of the first signal assessment of the safety and effectiveness of combined drug regimens in older age (the so-called „signal drug revision“).

The course deepens the knowledge of the participants especially in the areas of specific geriatric drug indications, the appropriateness of the choice of drugs within drug groups for seniors in various degrees of geriatric frailty, in specifics of geriatric dosing, geriatric dosing intervals, potentially inappropriate drugs/drug procedures in older age, explicit and implicit methods of the evaluation of quality of geriatric drug prescription, in the evaluation of drug-drug and drug-disease interactions and enables participants to practice basic knowledge in the form of interactive teaching materials (case studies and interactive tests). It contains 8 physical educational modules each with 3 professional lectures given by experts in geriatrics, gerontology, clinical pharmacy and rational geriatric pharmacotherapy. The professional contents of the lectures and interactive materials also include knowledge regarding pharmacokinetic and pharmacodynamic changes in older age and their impact on the changed effectiveness and safety of the most frequently indicated drug procedures, on the specifics of pharmacotherapy of cardiovascular diseases and diabetes mellitus in older age, on specific approaches in the treatment of dementia , depression, insomnia and pain, principles of adjusting drug dosages in different stages of renopathy in older adults, solving geriatric ion dysbalances, symptom management and specifics of palliative care for geriatric patients, as well as rational choice of various drug forms. Introductory and final lectures also include information regarding the aging of the global population, "ageism" (i.e. discrimination against older adults) in the field of effective and safe use of drugs, pharmacoeconomic issues of the need to reduce drug hospitalizations and individualization of drug regimens in older patients, and the importance of interdisciplinary cooperation in the effective, safe and cost-effective geriatric pharmacotherapy.

Through the presented knowledge and training of the first skills, the course enables "up-skilling" (increase in the knowledge, skills and competeces) of physicians, pharmacists and other healthcare professionals in modern approaches of individualized geriatric pharmacotherapy. Taking into account the fact that geriatric pharmacotherapy has its own significant specifics compared to classical middle-aged medicine, the course provides essential knowledge, abilities and skills to assess and adjust the individual effectiveness and safety of drug regimens in older age. The aim of the course is to increase the application of new, modern knowledge and skills in the real clinical practice and to strengthen the interdisciplinary cooperation of physicians, clinical pharmacists and other pharmacists in rational geriatric pharmacotherapy.
The physical lecture modules (a total of 8 modules running from September to June of the given academic year) always contain 3 expert lectures ending with interactive tests. The detailed professional content of the lecture modules is stated at https://www.faf.cuni.cz/Geriatricka-farmakoterapie/. Lecture modules are held in the Medicinal Plant Garden of the Faculty of Pharmacy, Charles University in Hradec Králové, in the main seminar room. For education, the course supportive materials are also available for participants in e-form on the course website, which are only accessible to accepted participants. These are interactive educational materials (tests, case studies), course syllabi in the form of an e-book, case studies in the form of an e-book, interactive tests in the form of an e-book, full lectures for an overview and course monography (2 parts) for self-study.Price for moography is included in th course annual fee.
A prerequisite for admission to the educational course is completed undergraduate education in the field of pharmacy, medicine or other health care fields at Czech medical or pharmaceutical faculties in the Czech Republic or equivalent education abroad. Knowledge of the Czech language (at least good passive knowledge) is required, as lecture modules and all educational materials are prepared only in the Czech language.
Applicants will be accepted to the course on the basis of proof of a diploma of completed master's education at the faculties of pharmacy, medical faculties or other health care fields in the Czech Republic or upon obtaining an equivalent education abroad. A certified copy of the diploma of the education achieved is sufficient as proof. The maximum number of accepted participants in one year will be 50, the minimum number for opening a course is assumed as 15-20 participants. When enrolling in the course, applicants will be preferred according to the date of delivery of the valid application and the date of payment of the annual course fee.
Knowledge (theoretical/factual): The graduate is able to describe pharmacokinet. and pharmacodyn. changes in older age and their impact on the efficacy and safety of medicines; describe the degrees of ger. frailty, drug-related geriatr. syndromes/symptomes, their diagnostics and practical use; to state ger. specificities of the medication choice, doses and dosing intervals and the length of therapy for the most common diseases (cardiovascular dis., diabetes, renal failure, dementia, insomnia, depression, pain, ion imbalances and in palliative care).

Skills (practical/functional): The graduate is able to roughly assess basic appropriateness of setting ger. drug regimens (at the level of a signal medication review), educate the patient about the risks and supportive measures; assess whether the severity of the clin. condition already requires the intervention of a clin. pharmacist or other specialists and refer the patient to appropr. care.

Competence: The graduate is capable at the level of signal medication review to assess basic clin., ethical, social and econ. aspects of rac. ger. pharmacotherapy (including issues of ageism), integrate them into own practice and proceed in interdisc. cooperation with clin. pharmacists and physicians.
